    various shifts (both weekday & weekends)

    Responsible for the supervision and case management of adult consumers living with mental illness in a long term residence. Responsibilities include supervising, supportive counseling, assisting with ADL skills, medication monitoring, PCA, facilitating social/recreational activities, and crisis intervention when needed.

    17 hours per week for each position (weekdays only between hours 9am-6pm, flexible)
    Bilingual in English and Spanish required.

    Provides support services for outpatient case management for ADULT clients in an office setting. Assesses client needs and help obtain services and support.

    ***Note that this role serves an ADULT outpatient psychiatric population.***

  • Morris Plains, New Jersey
    WEEKENDS 12AM - 8AM, 4PM - 12AM OR 1PM - 9PM

    Provides supervision and case management in a residential home for consumers diagnosed with mental illness. Responsibilities include assisting consumers with daily living skills, recording progress notes using electronic records, overseeing self-administration of prescribed meds, transportation to medical appointments when needed and performing crisis intervention, as needed. Functions as an active member of the treatment team.

    15 hours per week (3pm - 8pm)

    Drives an Agency owned vehicle providing transportation for adolescent teenagers to and from our Hackensack after-school partial care program. Assits with food preparation for evening meal and aides wiht therapeutic groups and activities. Must demonstrate sensitivity to the needs of our consumers. Ensures that the Agency vehicle is safe and properly maintained.

  • An MSW/LCSW Outpatient Therapist working in our Hackensack, NJ Outpatient Department (OPD).

  • Responsible for input of services and billing data for the CORE Solutions/In-Community program.

  • Seeking Psychiatrist to provide treatment to in a community mental health center outpatient setting for adolescent and adult consumers.

  • Performs highly diversified duties to install, troubleshoot, repair and maintain the facilities and their grounds according to all safety guidelines. Periodically performs custodial duties.

  • Provide outreach case management services to homeless individuals who are severely mentally ill.

  • Case management and supervision of adult consumers living with mental illness in an adult residential setting.

  • Acts as a liaison between the medical community and the staff. Educates consumers on health-related topics.

  • Responsible for securing and maintaining for all Adult Residential clients all the benefits for which they are eligible. Works with Program Coordinator to ensure maximum reimbursement rate of all benefits.

  • Provides initial telephone screenings for intake evaluation, crisis intervention, and information and referral. Schedules and assigns intake appointments for the Outpatient Department and Partial Care. Tracks all intake appointments and updates schedules as needed.
