Case Manager (FT)

Provide outreach case management services to homeless individuals who are severely mentally ill. Responsible for management of caseload, providing linkage to mental health services, medical and dental services, permanent and temporary housing as well as financial resources. Candidates must maintain professional relationships with local social service agencies, hospitals, and other organizations that may provide services to the PATH consumers.

Requirements: BSW in social work or BS/BA in psychology or a related field. Two years of direct experience working with individuals with a serious mental illness in addition to possessing extensive knowledge of local resources to aid the population of consumers served. Valid New Jersey driver’s license that is current and in good standing.
Shift: Full-time, Monday through Thursday, 9am-5pm, Friday, 9am-4pm.
Location: Hackensack, New Jersey.
