Case Manager (Full-time)

Provides case management and direct support services to consumers both in the program and in the community. Implements treatment plan, manages cases, secures service linkage, and provides transportation as needed. Must be able to work as an effective team member by covering shifts, participating in program activities and meetings, and communicating all relevant information to peers. Ensures all documentation is completed in a timely manner and consistent with established procedures. Utilizes supervision and crisis intervention as needed.

Requirements: BSW in social work, BA in psychology, or a related field. Must have two years of direct service experience in a residential or social service setting. Valid New Jersey driver's license that is current and in good standing is required.
Shift: Sunday, 9am-5pm, Monday, 8am-4pm, Tuesday, 12pm-8pm, Wednesday, 12pm-8am, and Thursday, 8am-4pm.
Location: Westwood, NJ.
