Supportive Housing Counselors (Full-Time)

Responsible for the management of consumer cases, providing IMR services, medication monitoring, and assisting consumers with financial management and linkage to all community resources. In addition, must be able to assist consumers with coordinating medical appointments, ADLs, and ensuring they meet all program guidelines. Must be able to encourage family involvement as needed, perform crisis intervention, and provide input into the development of IRPs. Functions as an active team member by assisting peers and communicating all relevant information. Must be able to work independently while out in the field and operate Agency vehicles in a safe and responsible manner.

Requirements: BSW in social work or BS/BA in psychology or a related field. One year of direct service experience in human services/mental health preferred. Valid New Jersey driver's license that is current and in good standing is required.
Shift: 40 hours per week, Monday through Friday with two days working 1pm-9pm.
Location: Hackensack, New Jersey.
