Supportive Housing Counselors (Part-Time)
Responsible for the management of consumer cases from a supportive housing program. Provides Illness Management and Recovery (IMR) services and medication monitoring to consumers. Responsible for assisting consumer with financial management and linkage to community resources. Responsible for coordinating medical appointments, assisting consumers with their activities of daily living skills, and meeting program guidelines. Performs crisis intervention as required. Facilitates family involvement where indicated. Provides input to the development of the Individual Recovery Plan (IRP). Must be able to communicate with peers and communicate all relevant consumer information to other team members. Must be able to work independently while in the field.
Requirements: BSW in social work or BS/BA in psychology or a related field. Experience working with adults with a chronic mental illness preferred. Valid New Jersey driver's license that is current and in good standing is required.
Shift: Part-time positions available.
