Full-Time Case Manager

Responsibilities:

  • Provides outreach case management services to consumers referred to the PATH program and evaluates their needs
  • Responsible for the input and development of treatment plans and ensuring accurate documentation of all critical consumer related information
  • Maintains individual caseload, assists and links consumers in obtaining medical, social, rehabilitative, and other services
  • Must be able to work and maintain professional relationships with local services agencies, hospitals, and other entities that provide services to PATH consumers

Qualifications:

  • BSW in social work, BA/BS in psychology, or a related field
  • Two years of previous experience in mental health and human services
  • Valid New Jersey driver’s license
