Full-Time Outpatient Case Manager

Provides consumers with intake evaluations, evaluates all concrete and service needs, and assists consumers in gaining access to medical, social, rehabilitative, financial, housing, and mental health services. Provides supportive counseling and co-facilitates support groups, coordinates and implements all treatment plans, and provides crisis intervention services as needed. Additionally, must be advocate for consumers with all external agencies, including state, county, and local government agencies.

Requirements

  • MSW in social work
  • Licensed Social Work (LSW) in the State of New Jersey
  • Valid New Jersey driver's license
  • Bilingual in English and Spanish desired, but not required
  • Shift is Monday through Friday, 4 days 9am to 5pm and 1 evening shift
  • Position located in Hackensack, New Jersey
