Full-Time Case Manager

Responsibilities

  • Checks in daily with assigned consumers on caseload

  • Holds family orientations and maintains weekly contact with families regarding visitations

  • Organizes recreational events with assigned consumers

  • Participates in all staff and treatment team meetings, in addition to weekly rounds with the clinical team

  • Coordinates discharges planning as needed

  • Ensures the completion of all clinical and administrative documentation according to the organization’s policy

Qualifications

  • BSW in social work, BA/BS in psychology, or a related field

  • Three to five years of previous experience

  • Valid New Jersey driver’s license
