Full-Time Case Manager
Responsibilities
Checks in daily with assigned consumers on caseload
Holds family orientations and maintains weekly contact with families regarding visitations
Organizes recreational events with assigned consumers
Participates in all staff and treatment team meetings, in addition to weekly rounds with the clinical team
Coordinates discharges planning as needed
Ensures the completion of all clinical and administrative documentation according to the organization’s policy
Qualifications
BSW in social work, BA/BS in psychology, or a related field
Three to five years of previous experience
Valid New Jersey driver’s license
