CBH Care Assists in Homeless Outreach in Englewood, NJ

CBH Care was among one dozen organizations, including police officers and social work outreach peer recovery specialists, to join together for “Operation Homecoming,” an initiative designed to locate homeless persons in Englewood, NJ and connect them with needed services. The participants were divided into two teams based on outreach and support. 

Alberto DeLeon, coordinator of CBH Care’s Project for Assistance in Transition from Homelessness (PATH) Program, joined the outreach team which also consisted of a police department supervisor, a CIT-trained police officer, nurse, social worker, and a recovery specialist. The team was organized to provide access to help in the form of ambulance services, addiction and recovery specialists, detox and treatment specialists, and other services with an eye on reducing recidivism. The support team was in standby mode in case needed. 

An Organized Effort to Help the Homeless

The outreach team worked together to locate homeless individuals based on guidance provided by the Englewood police department. “The goal of Operation Homecoming was to identify and engage with area homeless, and direct them to resources and opportunities based on team members’ areas of expertise,” DeLeon said. PATH is an outreach-based case management program providing services to the mentally ill homeless in Bergen County. PATH links homeless consumers with the services they need, including medical and psychiatric treatment, housing, legal assistance, and financial assistance. 

The CBH Care PATH program helps individuals 18 years old and over who have a primary diagnosis of serious and persistent mental illness or have a mental illness co-occurring with a substance abuse disorder, and who are homeless or are at imminent risk of becoming homeless.
